✔️ Indications

Provair 10 is a prescription drug indicated for the prevention of asthma attacks and relief of allergy symptoms such as runny nose and sneezing. It eases breathing by reducing airway irritation.

✔️ Pharmacology

Montelukast (CysLT1) is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor. Cysteinyl leukotrienes (LTC4, LTD4, and LTE4) are produced from various cells, including mast cells and eosinophils, as a result of arachidonic acid metabolism. These leukotrienes and the occupation of leukotriene receptors have been linked to the pathophysiology of asthma and allergic rhinitis, which includes airway edema, smooth muscle contraction, and altered cellular activity associated with the inflammatory process, all contributing to asthma symptoms.

✔️ Dosage & Administration

Adults and adolescents (15 years and older) should take 10mg/day for asthma and allergic rhinitis and 10mg/day for exercise-induced bronchoconstriction. 

Pediatric patients (6 to 14 years) should take 5mg/day for asthma and allergic rhinitis and 5mg/day for exercise-induced bronchoconstriction. 

Pediatric patients (6 months to 5 years) should take 4mg/day for asthma and allergic rhinitis. 

Patients with both asthma and allergic rhinitis should take only one tablet per day in the evening, at least two hours before activity to avoid acute exercise-induced bronchoconstriction.

✔️ Interaction

Montelukast can be taken with theophylline, prednisone, prednisolone, terfenadine, digoxin, warfarin, gemfibrozil, itraconazole, sedative-hypnotics, non-steroidal anti-inflammatory drugs, benzodiazepines, decongestants, oral contraceptives, and CYP enzyme inducers without changing the dose. 

✔️ Contraindications

Provair 10 should not be used by patients who are hypersensitive to any of its ingredients.

✔️ Side Effects

Common side effects include 

• diarrhea,
• fever,
• gastrointestinal distress,
• headache,
• nausea,
• vomiting, skin reactions, and
• upper respiratory tract infections.

Less common side effects include 

• akathisia,
• anxiety,
• arthralgia,
• asthenia,
• aberrant behavior,
• depression,
• dizziness,
• drowsiness,
• dry mouth,
• headache,

irritability, malaise, muscle complaints, edema, seizure, and sleep difficulties. Uncommon side effects include angioedema, disorientation, erythema nodosum, hallucinations, hepatic issues, memory loss, palpitations, pulmonary eosinophilia, suicidal thoughts, and tremor.

✔️ Pregnancy & Lactation

Montelukast should only be used during pregnancy if it is clearly needed. Provair 10 is excreted in breast milk, so caution should be exercised when it is given to a nursing mother.

✔️ Precautions & Warnings

Provair 10 is not recommended for treating acute asthma attacks with bronchospasm or status asthmaticus. 

Neuropsychiatric events such as 

• agitation,
• anger,
• anxiety,
• sadness,
• disorientation,
• attention disturbance,
• odd dreams,
• hallucinations,
• insomnia,
• irritability,
• memory impairment,
• restlessness, somnambulism, suicidal thoughts, and conduct (including suicide) and tremor may occur.
